Appointment Booking and Office Visit
To request an initial appointment or a follow-up appointment, please click the following "Book Appointment" Link.
If you are an old patient prior to June 30, 2020, please make an appointment as a new patient to complete medical questionnaires. Make sure you use the email address on the file to allow linking your record to the system. If you have been using this appointment booking system and have been seen by Dr. Wang before, then make an appointment as an existing patient.
For a new patient, please be sure to follow the workflow to complete your request. Most importantly if you intend to be an insurance covered patient, you MUST provide the insurance information for verification. If you do not provide insurance information, your request for an appointment will be rejected unless you intend to pay out of your own pocket. In that case, please indicate in the notes section of the request form.
Your appointment request will be reviewed and your insurance information will be verified within 48 hours after receiving the request. A confirmation email/text will be sent to you to confirm the appointment. Before receiving the confirmation, you appointment is not scheduled.
Once your appointment requested has been accepted, please follow the confirmation email to "Manage Your Appointment" by completing the onboard process. Please make sure you read & sign electronically at the very end after consenting all the required forms. If you don't sign the forms prior to coming to the clinic, you will be asked to sign them in the clinic before your treatment can be proceeded.
